Services

The services to be provided by the coach to the client are Zoom/Skype coaching, as agreed jointly with the client. Coaching may address specific personal projects, business successes, or general conditions in the client's life or profession.

Other coaching services include value clarification, brainstorming, identifying plans of action, examining modes of operating in life, asking clarifying questions, and making empowering requests or suggestions for action. Throughout the working relationship, the coach will engage in direct and personal conversations.

The client understands that successful coaching requires a co-active collaborative approach between client and coach. In the coaching relationship, the coach plays the role of a facilitator of change, but it is the client's responsibility to enact or bring about the change. If the client believes the coaching is not working as desired, the client will communicate and take action to activley participate and engage in the coaching relationship.

 

Prior History

The client also agrees to disclose details of the past or present psychological or psychiatric treatment. In entering into the coaching relationship and signing the agreement, you are agreeing that if any mental health difficulties arise during the course of the coaching relationship, you will notify Intuitive Healing and or your Practitioner immediately so that we can discuss with you appropriate steps and if any additional referrals are needed.

 

Privacy

The client can, at any point in the coaching session, declare his/her preference not to discuss a specific issue, by simply stating that they would rather not discuss this issue. The coach agrees to respect this boundary and will not attempt to forward the conversation further along those lines. By signing this form, I consent that Intuitive Healing may release information to a specific individual or agency if it has been determined that a vulnerable person (child or adult) is at risk; if I, as a client, am in imminent danger to myself or others; or if a subpoena of records has been requested.

 

I also understand that, at any time, Intuitive Healing may discuss aspects of my case with other colleagues, keeping my full name and identity completely confidential always unless I have given permission otherwise.

 

Confidentiality

All information about the coach / client relationship will remain strictly confidential except in very rare circumstances where decreed by law; ie. where the court might issue a subpoena for the file or information. If you wish for Intuitive Healing or your coach to speak to someone outside our interactions, then you need to provide written permission (original letter or email) to do so. Exceptions to confidentiality of course relate to circumstances such as intent to seriously harm yourself, another person, child abuse etc. Otherwise, all your information is confidential.

Please note that in some situations, it is important to be aware of the use of technology in that for some clients, there is a risk in using certain media such as the internet, mobile phones and cordless phones. If you use these to communicate with me, then I will assume that it is appropriate to continue to do so in our interactions with you.

 

Forfeiture

As the client, I understand and agree that I will book my first session within 30 calendar days of the purchase date. I further understand and agree that if I do not book a session within that time period and I do not submit a request in writing to a member of Intuitive Healing for help or guidance on booking a session, I forfeit all monies and remaining sessions. I also agree that I will book all sessions as the selected service or package intends.

i.e. If a 4 week package is purchased, then the client will book 1 session per week. Unless there is prior agreement in writing.

 

Fees

All fees are paid for in full, unless specified by a prior written agreement.

Additional appointments can be scheduled as needed. If you need to reschedule an appointment, please provide at least 48 hours’ notice. These fees will be paid in advance and are non-refundable.

 

Termination

Coaching under the terms and aforementioned agreements will continue for the duration of the contracted period. Only under extenuating circumstance can our coaching agreement be postponed up to 6 months of the said agreement.

Your acknowledgement to this agreement indicates full understanding of and agreement with the information outlined above. To be clear, completing the purchase of this or any product/service by Intuitive Healing, indicates and solidifies your agreement to these terms.

 

Audio Recording(s)

I give Intuitive Healing full permission to make audio recordings that may include my voice. I understand that if a recording (or recordings) is made during or after my session(s) Intuitive Healing retains full copyright over any forms of media that may be produced and distributed to me.

 

Liability

I, (The Undersigned Client), hereby release Intuitive Healing and all practitioners here in from any and all liability or claims that could be made against (him/her) concerning my mental and/or physical well-being during the work that has been outlined and agreed upon (now and in the future) by agreeing to this form. This liability waiver is not intended to exclude or restrict liability for death or personal injury caused by negligence.

Scope of Practice

I understand that Intuitive Healing is not a licensed physician, psychologist, or medical practitioner of any kind and that services provided by Intuitive Healing should not be considered a replacement for the advice and/or services of a psychiatrist, psychologist, psychotherapist, or doctor.

 

Guarantee

I understand that although Rapid Transformational Therapy® has an incredibly high success rate, Intuitive Healing cannot and does not guarantee results since my own personal success depends on many factors that Intuitive Healing has no control over, including my willingness and desire to effect the changes inside myself.
